What the report contains
- the mine: ID, name, status and date, type, commodities, operator
- employment and hours reported for the last four quarters
- inspections, citations and accident reports since October 1, 2024
- what was not found, written down: the checks made and the empty results
- the exact citation: MSHA file and date

What the report is NOT
It is not a document of MSHA. Counts are those of the file at its date; penalties are proposed amounts and can be contested.
This is an order form. The request reaches us and we confirm it by e-mail before any payment. Nothing is charged at this stage.
